An interdisciplinary care plan team will work hand-in-hand with each resident's physician to develop a plan of care for your loved one. Their purpose is to create a plan of care which is tailored to each resident's individual needs. The interdisciplinary care planning team, which consists of nursing, social services, activities, dietary, and rehabilitation meet at least quarterly to review each resident’s progress and determine the proper course of action or “plan of care”. We strongly encourage families to attend and participate in this care planning process, as your experience and knowledge of your loved one's interests and activities can greatly improve this process.
